Season Three And Lifestyle Medicine

SPEAKER_02

Young buddies, welcome back to season three of the podcast. We'll be talking about all of the different pillars of lifestyle medicine from nutrition to exercise to stress to sleep and everything in between.

SPEAKER_03

Hey, this is my favorite conversation that we're going to have. Because I've had a conversation with a colleague of Ecos Christelle Thériot, chirurgienne bariatrique, Dr. José Lantin Dupuy, psychologue and colleague of me at the UMF. We talk about the professional of the santé, but also the public in general. The environment, it's just a problem of discipline and willpower. I'm going to talk about the medication. The medication has been super popular in the years. We go V, Zbound, Manjaro, all these medications. So the message that we have envoyed from professional about the obesity, comment the medications have their role to do, so after we find this, I'm parle of your parcours professional for the people you connect.

Meet The Bariatric Surgery Team

SPEAKER_03

Christelle, te sensor to partake the View, piscine, tonement professionnel.

SPEAKER_04

SPEAKER_03

Excellent. Merci de Becny, hein. Evidently, ton expertise is not rare to be a chirurgic in general, but we have a fellow in chirurgic bariatrique, it's an extra layer of expertise. So I'm fired that our students have access to this information on top. Joseph, we've got a couple of days.

SPEAKER_00

I'm originaire of Dieppe. I've had my backup special in psychology, I've had my doctorat professional at the University of Moncton. I've been here seven years in different departments, but just at the clinic bariatrique. This is where I'm present. This is where I'm thinking I'm in a role in the world with the residents in medicine of family.

SPEAKER_03

They're coaching lifestyle, they enter pas obesity, alimentaire.

Why Obesity Is Not Willpower

SPEAKER_03

So j'aimerais, genre definition formelle, c'est pas ça que je cherche, but explique aux patients who are in consultation what's what l'obesity? Is it just an IMC? Is it just a shift on a balance? No, it's so complex.

SPEAKER_04

The patients are judged and they arrive at the clinic, they are content because they are equitable and comprising one. It's a little what, but it's not just that. It's not just who manages and passing exercises. It's not just that. It's certainly a composant genetic that is important, a composite genetic that will influence the hormones, the metabolism, who are implicated in the obesity, a composite social, psychological, socio-economic, psychological, and a great part of hiatrogenic by the medicaments. Different causes, a lot of causes that are modified. It's in the genetics, it's in the hormones, it's in the metabolisme.

SPEAKER_03

It's a man, the medication, your psychology, your environment, the events of ta jeunesse, des stressors in the day of the year.

SPEAKER_00

It's that will result the points. But it's a simplification. The problem with that, it's that it's service to person because we are enough to pass to offer their traitors that are efficacy.

SPEAKER_03

It's a problem of motivation. Exactly. He does influence by what he is on Facebook or Instagram, he does at some point interior that it's his fault, that it's just that he has willpower. He will, or probably assume, a long rendezvous multiple consecrated to comprend the multiple factors that have made a point that the patient is already walk me through from kind of my rendezvous or resume l'emball of the rendezvous.

The Psych Evaluation That Changes Care

SPEAKER_03

J'aimerais to comprendre what a psychological has a role important to do in the clinic of obesity.

SPEAKER_00

Exactly. But the first context is in the context of an evaluation psychosocial. It's to determine what you said, to compromise the patient, to be capable of what their force is, what are the things that we can use from a level to have success, but also to know what they would potentially have a difficult thing for the patient that will interfere a little bit with his progress, and his results in post-operation. But we also have information on the patient to ask just to avoid these bills or these myths that the patient would arrive with. Because when they went to the clean bariatrique, they have asked to manage more, bought more, they were not to consider a chirurgically. But it would be like an exhibition. I think it's that, I've had that, but I've had the results that I cherry. I've had an exhibition. Exactly.

SPEAKER_03

Demoralised, impure, victim. I imagine that it depends a bit an identity, the manner the view tour. The patient measures the success from the shift, ah, it's sort of. Correct me if I'm wrong, but it's probably no win. Exactly. Come on, measure the success on bariatry.

SPEAKER_00

When we have an intervention comport, it's like the patient has a control. We can do it. The number of efforts that the patient will make will be proportional to his results. He has an effort in the march, he has a march excellent and success. But if the 20 lives the patient will make attention to what he does, he will make exercises, he will make an effort.

SPEAKER_03

Okay, so he pays, he has an exhibition about these efforts that you have.

SPEAKER_00

Exactly. Because it's like we perhaps self-efficacy. It's like the control on what I have. But if we are constantly in a position where it's going to perform this crown that I can change my life, I'll change my children, and it will have an effect positive.

SPEAKER_04

Don't worry about the pain. After the church bariatrique, eux who visit their chiffre and vise a shift précision, I'm perdre tant de poids, pis they regard to the chiffre sur la balance. Fait que on essaye beaucoup plus to mettre le succès, not on the shift sur la balance, but améliorer their quality of view, comment améliorer their problems of santé aussi.

SPEAKER_03

A patient fed de meilleur, hein, beaucoup de circuits and comportements changes? Is that it changes after the chirurgically bariatrique? I'm sorry, like the chirurgic specifically in the second, but the patients that you say after the chirurgic.

SPEAKER_00

Exactly. I think Christelle will parler, but no.

Emotional Eating And Post-Op Risks

SPEAKER_00

It's like the intervention psychological in pre-opération is important.

SPEAKER_01

It's very important, okay.

SPEAKER_00

Because if the alienation is a method of coping to live with emotions difficult, and we have an method of coping, we read the chirurgic bariatrique, and we are attending to change a little bit these habitudes, a little bit prior to you. After the chirurgic bariatrics, there's an augmentation of the risk of trouble of consummation of substance. Evidently there are processes that are a nice inconscient, but it's a little bit what happens. It could have an exacerbation of symptoms, because we say, for example, that the risk of suicide augmented in post-opération.

SPEAKER_01

Okay.

SPEAKER_00

It's an enormous changement. Are you adapting that utility enough of our resources? In the recuperation physics, post-stop, sure, the recommendations that are, the changement of their corps, their appearance.

SPEAKER_03

The stigma to have passed in a chirurgically.

SPEAKER_00

Is it their affairs? Is it mentioned if you don't say it? But the question of identity or image corporate.

SPEAKER_03

Wow. Super.

SPEAKER_00

It's a little simplified to explain. But okay, it's capable of developing other methods of coping, to have a process emotional to be capable of face to emotions that are difficult, sort of consuming elements that are on suicide, on grasp, on cell.

SPEAKER_03

So when we say the emotional eating, it's like the problem is necessarily completely regular Dr. Crystal and Chirurgic Bariatrich, because the girls are staying playlists. I imagine that at Stuart we have this by laparoscopy.

How Bariatric Surgery Works Today

SPEAKER_04

Last sleeve, which is interesting, but it's the chirurgically. In France, I appear so manchant, but it's like a sleeve. It's like an estomacy on the long. We enlave a three quarter of l'estomac, environment, two or three quarters of the estomac at this moment.

SPEAKER_03

The mécanisme is just that it diminues the volume of the estomac?

SPEAKER_04

It's interesting because the ghreline is that I explicitly produce principally by the estomac. Tandis qu'avec la ben, parfois the patients will souffrir de la faim.

SPEAKER_03

So you parle de ghreline, leptine, c'est quoi ces affaires-là? Il y en a beaucoup des hormones.

SPEAKER_04

SPEAKER_04

L'estomac, c'est un muscle. C'est sûr que au début, the réservoir gastric is very petit. Moi, j'ai toujours imaginé que l'estomac était like a grosse balouune, maintenant it's a banane. C'est literally.

SPEAKER_03

Alors qu'avant, c'était 3-4. C'est plus, oui.

SPEAKER_04

Donc oui, there's a restriction. But the satiété, it's those hormones that the world because it's the famous GLP1. The GLP1 is the satiety that the appétit. Fait que toute one who is Ozempic, Wegovie, etc. Each will go into the satiety. But in boot, when you diminish the appetite or augmented, it's a little result.

SPEAKER_03

José vient just to say it's complex.

Hormones Hunger And Satiety Signals

SPEAKER_03

Parce que dans le gastric bypass, puis the other types of bypass that exist, we interfere with the GLP1 aussi en bypassant. Parce que le GLP1, it's an hormone intestinal. Don't do a fin physique, the finish. La fin comme the volume of l'estomac, la restriction. D'accord, d'accord. Come on the famous contrable bipropion, which is a médicament until psychological.

SPEAKER_03

To put this risk low to operate. The process of suivi, supplément après, c'est. What is taux de succès?

Defining Success Beyond BMI

SPEAKER_03

Oui. On a une idea of the factors that can agree.

SPEAKER_00

I think there's a lot of elements, like I said, but I think there's an element that changes rapidly, it's the environment. We have a click, we'll see. When we regard the television, what's announcement in soirée, it's a hamburger, and a fruit. I think this is an element that has told rapidly that explicitly l'obesity augmented with our population.

SPEAKER_03

The nourriture we are with has changed drastically.

SPEAKER_04

It's super interesting parce que souvent and I parent my students, I think if we want to three, or long time, they haven't facilitately access to the nourriture. Now the corps is fit that we could survive in jeune phone. It's necessary, but now it contributed a little obesity with access to the malbouche.

SPEAKER_03

Exactly. The famine arrived. Exactly. It's an advantage metabolic. It's survey. Exactly. But we have an environment of abundance that we have.

SPEAKER_04

Now the corner before you poi. Because there's really mechanisms that won't install. After you perhaps the corner with it like a stress. If you have 25, 20 or 50 lives, they will know that a stress. Mm-hmm. Fait que the corps will make in place des mechanisms d'adaptation for that you arrêts to perdent dual poids. C'est là que la chirurgie barrique is interesting, and the medicaments are interesting, it will aid these mechanisms. It's a mechanism of defense that ton has, tongue ta GPL P1 va baisser. When you perhaps, ton metabolisme ralentis to empêcher to perd du poids. Fait que des fois it's important, okay, the exercise, but the metabolism of base ralenties are quite a great part of the dependence energetic in our journey. It's sort of en faisant exercise en augmentant la masse muscular, that will aid. D'où l'importance des habitudes of vie.

Environment Set Point And Metabolic Pushback

SPEAKER_04

100%.

SPEAKER_03

The quote I'm using, is you want just to master an equipment interesting. It's like your stress. This genre of thermostat that we have. The concept of set point theory.

SPEAKER_00

I think it's very evolutive. A drive motivational to church, our car to guard the energy that it was consuming to depensive. It's genial in this time-là, but it's just that now the environment has tells me that it's not the way. We have to generalize, but in general, the problem is that the nourriture is not disponible, it's that it is too disponible. It's like our elastic plus en plus. What is it at our point of department, which is like a set point or initial.

SPEAKER_04

It's pretty and anticipated. It's like it's important to see because the girlfriend will panic when it will arrive. But the changes hormonal and metabolic that will install, because they have points. It's like the faith will be a little bit the cravings, the metabolism will ralentice it's difficult. Don't after earlier, my patients will say after the 500, it will be facilitated to maintain their poids. I've said to remember if you have a bonus who étudia After the set pointed poids santé, after a perfect poison. It's not a shift exact, but it's certainly one or two. After one or two, it's maintenance tonight to change this set point. Probably autour of 50 or more, pas just one or two, parce que it's more facilitating to reprend the poids, is that?

SPEAKER_03

The convenience of not besides ménagers that brûle, because we talk about energy, we are the metabolism of base, we'll see the calories brûling, the exercise. Just to process, diger, and absorbing nourriture.

SPEAKER_04

Exactly.

Food Choices That Quiet Hunger

SPEAKER_04

SPEAKER_03

Les gens disent ⁇ Exactement. On parle beaucoup de faim, on parle beaucoup de single de satiety, on entend beaucoup the term food noise, craving. Two avenue des GLP1. Comment you definirais food noise for a dish, or for a compression. Parce que the girl that a while they commence these new traitements for the perte de poids, you're gonna food noise s'en va. So comment expliquer ce food noise?

SPEAKER_00

It's a good question. Because I think we've got allusion to the fan psychological, the fan physics. But I'm just presenting from quite different types of fan. Okay, so we have a fan physiological, I'm applying the fan bidden. I'll be talking about my pet who has six answers.

SPEAKER_02

It's the stomach!

SPEAKER_00

It's when you think that physiquement your car has nourished. It's in these situations that it's appropriate to manage.

SPEAKER_01

Yeah.

SPEAKER_00

But it's not the same situation that we ported to manage. Another type of faith, it's anything from our senses. Just chocolate in particular. We say exactly what. But it's also another sensation. Not controlled, but we arrive là, pis it's popcorn. But it's not a faim physiological. But it's really queen the senteur that appétens-là, that we don't have a go to many popcorn.

SPEAKER_03

Too.

SPEAKER_00

There are associations that we can make. It's like an announce, if it's a great affair of chemicals, it's an annonce at the television, but when we have certain aliments, it does not have enough to manage.

Food Noise Cues And Shame Cycles

SPEAKER_00

But there's also another fan that is the faith of hair or the faith emotional. We valorizes a lot of the emotions that we say positive. We don't feel, we have to be honest, connect with the other, it's what we cherry, but we have a little bit of difficulty, I'm society, to accept the emotions that we say uncomfortable. We don't think so. These emotions are normal to be like that. Exactly. It's a little bit like the patate chaude. It's like an emotion that is really comfortable. In these situations-lone, we have tendance to dirige versus a method of coping that at long term. Excuse me, for example, to make aliments that are grasping, like we said, it stimule the play of the dopamine. But the problem is that we don't have to address this emotion that we have. But the distress initial is already there, because we're not addressed. But in this childhood difficulty, like the culpability, the honte, the regret, because we say I've managed that. It's really what I'm saying. It devient a genre of service that alimente with the culpability that is.

SPEAKER_03

I assume that because these emotions are negative, not only they reach for a pom. It's not necessarily a more solution.

SPEAKER_00

It's that stimulus of plaisir of the service, it will not relate to dopamine. Ça veut dire que it's not the nourriture that's the solution to comment.

SPEAKER_03

It's that's the cause of this emotion negative. Exactly.

SPEAKER_00

Right. I'm trying, I'm fashion, I'm content. Because if we have a process emotional, I'm not capable of distinguished. But if I feel already, but I don't know what I'm very out there for me to think more. Because if I'm trying, I'm not beside the memory that if I'm flashing, right? So I say I'm fashion, but I mean I devrapic, I devraise, I devraise reconnaissance because my life is well. But I'm culpability or that I accept that I have this emotion, what is intense. It's like I said I would say an ours blanc. Okay? I'm just 10 seconds, pense pas an ours blanc. Like a couple, you can have other than an ours blanket. It's exactly the same with the emotions.

SPEAKER_03

Exactly.

SPEAKER_00

You see your emotions, you try an necessarily a solution, but you try to feel that.

SPEAKER_03

So the money food noise is used by the world, it's a very simplistic concept physiological, emotive, psychological, social.

SPEAKER_00

Because the tip of it, if I remain my habit. So if you imagine a person type, at eight hours, it's a moment for you to relax.

SPEAKER_03

SPEAKER_03

I've got that with all sorts of comportements. I've got a patient who must say to fumer, pis to dunno I'm here in the magazine, I'm sorry, I'm achieved cigarettes, I'm assisting my charge, I'm a luminary, I'm snapping out of it the early 5 minutes, and it's a blur. They have to fit comportments ultra complex. It's totally autopilot, right?

SPEAKER_00

It's the pilot automatic. The first is capable of recognition these situations-là, parce que même si on a l'impression, like my patient to just say, I'm there with my cigarette. In fact, it's plenty of étapes that have been passed. Wow, okay, what's this? Or is it a declench to this cascade of comportments and pensions that they're apported? Is that plenty of tendance to have the habit? Is that a tendance to hide when I hear these things? Is that emotional? But when we are capable of remarkable, if we change it, is to remark what it seems because we come to make a distance or a espace on the comportment. So we're ralenties, we remark, and we are advanced in our process. So it's really the first thing we would be capable of there.

SPEAKER_03

Super interesting.

GLP-1 Meds Done The Right Way

SPEAKER_03

It's just like the volume is based. Parlons of these medicaments-là, okay, because it's the internet is a wild place. There are gens who displayed that the chirurgic bariatrique exists because of the medicaments that are surprised, which are new, but passible that. Là ensuite, l'evolution is the demi-view of the molecules that we use for perhaps augmented, the piqûre are more frequent, it will transition version of the pilules. The per de poids that don't have n'importe quoi de 15 or 20-25% with the moon. So it's sort of beneficial. Come that work? What's the rules that you think that these medications-là doit in the gestion of the obesity? It's a médicament that you utilise at the clinic. But for a reason, on met les agonistes GLP1 or GLP, les analogues GLP, GIP, the Wigovie, Oz MP, Munjaro, the family of medicaments, they're like a category that is separate. Parce que je crois qu'on considère pas encore l'obesité comme une maladie chronique, on la voit comme différent than all these other conditions chronique. So, what's your experience with these médicaments-là, in terms of parts?

SPEAKER_04

Souvent, moi, when I parle de ça, je parle from des médicaments. But on devrait voir un peu le zemping of the same faith, or the munjaro, ou n'importe quel, wag of visit bound, etc. Donc, it's souvent that the population who are predisposed to print plus de poids for different raisins génétiques and métabolics will manque of these hormones-là.

Exactement. But then au Canada, I think that design, these medications-là are prescribed too vice sans don't have a lot of explicit. Or it will have too much effects secondary parce que it has been suivie and titrated along the effects. It's not me who's prescribed, it's just Dr. Nadine Roy who has been supplementary on the gestion of the obesity from the point of view medical. It's been bad. Alors it aurait function if it had commenced a pretty petite doses pendant.

SPEAKER_03

But là, ils vont tous malheureusement souvent reprendre ce poids-là.

SPEAKER_03

Ces gens-là have du coaching, they rencontre probably a psychological people. Dietetist. Dietetist. Exactly. The objective is not just to manger more pis perdre du poids. It's to reconnaît what you manages in certain situations.

SPEAKER_04

Exactly.

Access Support And Destigmatizing Care

SPEAKER_04

The accident is very limited. Fait que même a medicine a toute la bonne volonté, hein, explique bien toi, son patient, there's not a lot of accident to a psychologue, a dietitian, a health coach, or these patients who've just manquaid.

SPEAKER_03

But it is that these medicaments-là are prescribed almost. The patients demand count. So I think there's a pressure for the medicine to prescribe. We have a criteria very close and pre-étably on what they beneficient. I think we'll have the benefits and the risks of the passenger. Maladies cardiovascular, diabetes, cholesterol, hypertension, cancer are directly in lien with the obesity, which is a factor of risk principal. And the risks of the medication, obesite, the risks of the case.

SPEAKER_04

Ah okay, and you've got an effect secondary, but it's a very good thing. The nausée numérique.

SPEAKER_00

Because we have this concept that it's a question of motivation or it's like an exhibition moral difficulty to generate your poor. I think it's a favor to person to make sure that these stereotypes exist, because they are very present. It is just like this idea that if you're motivated, you seriously capable to pay. It's simple like that. Motive-toi, vas-y, you will have a success. But if you have that croyance-là, the fact that you're not capable of several is the idea that it's an exhibit. It's a false crown that will aliment the idea that you don't have a medication, you devraphy of a chirurgic, you devraise too.

SPEAKER_03

At the base, it's a man of comprehension of all the mechanisms in the fact that people live with an obesity.

SPEAKER_00

Sure, we have a patient who won't think about the potential to obtain a treatment. There's a culpability that is.

SPEAKER_03

Exactly.

SPEAKER_00

Exactly, stress.

SPEAKER_03

100%. 100% d'accord. It's a podcast lifestyle, medicine, the medicine, the mode of view. The objective is to create a conversation to shift the perspective into a culture of prevention, a situation privilege, to enseign the obesity to their future medicine. A co-op of medicine who have access to a curriculum of medicine in the mode of view for the enseignment that we have already reflected on what we can offer our future medicine. We're the mission, I think, to ameliorate the santy of the population, the santy of the public.

SPEAKER_04

It's just an outer cercle vicieux that they are dedicated. Maintenant, the médicaments font partie aussi. Puis for a patients, the médicaments are not sufficient. Parce que des fois, une perte de poids of 20%, it's passe pour avoir des benefices sur la santé and the maladies associated with the obesity. Don't think the chirurgic barotrique will encourage a place for these patients with designers. Exactly. But for the medicine, I think for all the medicine or the future who will référence en clinique baratrique, I think it's important that they have a discussion with the patient after the référer, to be sure that the patient will put enough main, puis essay to make, and he comprends that it's an out that will aid, puis that it's a quick fix. Parce que des fois, the population has this false croyances. About multidisciplinary. But despite that quick fix quand tu vas au privé. Parce que the monde will just pay for the church, they will have the church. C'est pour toute sort of different reasons complex aussi, c'est pas c'est rarement la faute of patient. It's rarely the fault of the patient.

SPEAKER_03

Y'all a lot of moving pieces. Okay. Chacun of these authors have an out that ages on this mechanism. But it's clear that it's an approach multidisciplinary. We have to have a chaise for a dietist already. I think it's ultra pertinent to have some perspective.

SPEAKER_00

We have tendance to be like a person over there who judge, but we have certain judgments on different things. So if you have a reflection honnête with our proper stereotypes that we can enter, at this moment, we are out there to agree differently. But if it's semblant that it exists and it's not, we don't have service to person. But it's human to have those people to accompany and aid.

SPEAKER_03

I think we have to simplify a maladies chronic that impacts a lot of people and all the complexities of sujet. But I think the important is on parle. The fact that we're sought to be capable of months that are surveilled, our colleagues. And it will absolutely influence and many that those will approach the conditions medical with the patients. I think it's destigmatizing, encourage the patients to their medicine, encourage the medicine to be receptive to party.
